Performance Appraisal Form – What is it?

The employee performance appraisal form is one of the performance management tools used in the employee performance planning and evaluation stages of the employee performance management process under the organization’s performance management system. In contrast to subjective performance appraisal forms, employee performance appraisal forms are objective in nature with well-established quantitative performance indicators.

The employee performance appraisal form consists of 2 parts

1- Objective Record Sheet (ORS) – This employee performance appraisal form is completed by the employee at the beginning of the year during the performance planning phase and serves as a basis during quarterly performance appraisals and semester reviews. It comes in Excel format with two major sections, the first is objective indicators which include measures in aspects of quality, quantity, time and cost. At the other extreme of each scale are corresponding ratings from excellent, good, fair and poor. Another major section is activities that cover essential activities; closing period; cost; Support is required and; Sources of support. After a critical consideration, the ORS is signed by both the employee and the immediate supervisor. An ORS will be completed for each KRA and target/strategy.

2- Individual Operating Plan (IOP) – Similarly, employees will complete this employee performance evaluation form during the planning phase but will be used more fully at quarterly and periodic reviews. Also, it comes in Excel format consolidating the entries of ORS. The form has 3 major sections, the first of which is Performance with an equal weight of 90%. These include supported institutional strategies/goals; Key Result Areas (KRAs); Weight per KRA; personal objectives; Assessment for ORS and; Final assessment. The second is the behavior section which accounts for 10% of the weightage. This includes both the employee’s self-evaluation and that of the immediate supervisor. In contrast to them are entries in the display of organizational core values; Feedback from clients and colleagues and; average rating. The third and final section of the form is the overall rating which includes the overall weighting; formula; weighted ratings and; Overall numerical rating. Typically, the IOP is signed by the employee and approved by the immediate supervisor.
